Focus on keeping seniors engaged in social and community activities. Includes transport assistance, light exercise support, and health management guidance.

How Support At Home Changes Aged Care
Previously, many home care packages were divided into fairly rigid levels. With Support At Home, the new approach puts seniors at the center of care planning. This means each person receives services based on their health goals, level of independence, and actual living environment.
